The Complex Case Manager (Continuing Healthcare - CHC) role at NHS Gloucestershire Integrated Care Board involves managing the care of individuals with highly complex health and social needs. The Band 7 position requires a registered professional to work autonomously within a supportive team, providing case management for a diverse patient population that includes those with physical health issues, learning disabilities, and mental health conditions. Key responsibilities include monitoring high-cost care packages, making decisions based on the CHC National Framework, attending court for case management, collaborating with care providers, and safeguarding vulnerable individuals.

This position offers opportunities for continuous learning, hybrid and flexible working arrangements, and the chance to make a meaningful impact on patients' lives. The role requires the completion of various administrative tasks, including the transfer of care packages to Personal Healthcare Budgets and the writing and reviewing of support plans and risk assessments. The post holder will ensure that patient-centered care is at the forefront and that all case management processes adhere to best practices. The job entails working 37.5 hours a week, Monday to Friday.
