The clinical lead will participate in and influence efforts across health and social services to shape multi-disciplinary pathways designed to support patient choice, improve quality of life, promote self-management, and assure early intervention through proactive provision of care in the patients' own homes.
The successful applicant will have a professional registration, have experience of working in community health or social care, be proficient, knowledgeable, and competent. Experience of team leadership and working across multiple organisations and IT systems is desirable, but all training will be provided.
Access to a vehicle and possession of a full driving license is essential.
Enhanced Recovery at Home Service, or ERS, is a team of health care support workers, assessors and care coordinators who provide care/support packages which are recovery focussed and support people to regain and/or maximise their independence. The clinical lead for ERS aspires to provide a responsive, safe, effective and caring service to patients requiring care/support/reablement including personal care, simple meal preparation, general confidence building and signposing to available support services including third sector, community health and social care services. The service operates from Aerodrome House, Gosport, with a satellite office in Havant Health Centre and delivers services to people living in Fareham, Gosport and South East Hampshire. ERS work closely with urgent community response teams, community nursing and therapy teams and with colleagues in Hampshire County Council to support admission avoidance and complex discharges, optimising individuals' functional needs and independence. Through comprehensive training and competency programmes, ERS staff are able to take physical observations and recognise risks to skin pressure areas. This enables the team to quickly identify a deteriorating patient and support them to access the appropriate service.
The phlebotomy team is a domicillary service across Fareham, Gosport and SE Hampshire collecting blood samples on behalf of the community nursing services and routine as part of the INR service.
