Trust Doctor in Intensive Care East (Cardiothoracic ICU) Anaesthesia with Imperial College Healthcare NHS Trust in London
The Hammersmith ICU-East (previously Cardiac ICU) provides Critical Care support to Cardiothoracic services delivered on the Hammersmith Site as well as other major surgical specialties like Hepato-biliary and Pancreatic, Oesophago-gastric surgery etc. The ICU-East has 16 beds which can all be used for Level 3 care. The cardiac anaesthetist/ intensivist consultant of the week is responsible for all clinical and non-clinical decision making in the unit. The post will also be suitable for those with a background in surgery (particularly cardiothoracic surgery) and medicine who wish to gain experience in ICU and learn the associated practical skills. There is an active programme of research within the unit as a whole, and the rota may allow for research and audit interests to be accommodated. We are committed to trying, to support the post holders with their individual professional needs by tailoring the rota and clinical exposure accordingly. The current work of the cardiothoracic department includes coronary artery bypass operations (on and off pump), valve replacement or repair and aortic root surgery. There is also a busy interventional cardiac catheter lab performing procedures such as ASD and PFO closure, MitraClips and TAVIs. Cardiology provides a regional PCI service which includes out of hospital cardiac arrest referrals and an emergency arrhythmia programme which is attached to the very busy electro cardiology unit and a pulmonary hypertension service. The successful applicant will be allocated to the most appropriate rota depending on their skills and experience. Flexibility will be appreciated. The expectation is that the post holder will: o have no other roles anywhere else in the hospital o be expected to stay on the unit for the duration of the shift o be part of the intensive care multi-disciplinary team o take an active part in consultant ward rounds, including presentation of patients o be responsible for the ordering of pathology and radiology tests and the collation of results. o as resident, be the first medical responder to clinical queries and problems related to patients and raised by the nursing staff o be expected to request senior help and support from the anaesthetic or cardiothoracic registrar or consultant. o be expected to attend the weekly teaching provided on critical care topics together with the general ICU residents - delivered by anaesthetic consultants and intensivists o be expected to gain competencies in critical care clinical scenarios and practical procedures, under the mentorship of the anaesthetic consultant. o gain a solid grounding in critical care and post-operative medicine and attend the ALS course o receive CALS training in a scenario-based approach on the unit as part of the multidisciplinary team About us Dedicated time to pursue other training interests including experience in research, audit and clinical experience in complementary specialties (Anaesthesia, Cardiothoracic Surgery or General Intensive Care) will be accommodated on request within rota hours (depending on clinical activity within the department) in the form of dedicated training weeks allocated within the rota. These have proved extremely popular amongst past post holders who have used these to advance their career gaining valuable experience in their chosen fields. Teaching Activities ( subject to variation): Tuesday 14.00 - 15.00 Anaesthesia trainees teaching session Wednesday 15.00 - 15.45 ICM journal club Thursday 07:45 0830 Cardiothoracic Surgery teaching Thursday 12:30 13:30 ICM teaching Clinical Governance Meetings Bi-monthly Morbidity and Mortality Anaesthetic Department Meetings and Cardiothoracic/Cardiology meetings. Duties and responsibilities 1) Clinical Responsibility for the day-to-day management of all patients in conjunction with the Cardiothoracic Intensive Care Consultant and the referring consultant and his staff. A full ward round is held each morning, and evening, including holidays and weekends. 2) Administrative Normal documentation of history and results etc plus entering some data on a computerised audit system. 3) Research An active clinical research programme is conducted in this unit. The clinical commitment of the resident is heavy but he/she will be encouraged to participate in any projects if interest is shown. 4) The post will be appointed for a period of 6 months initially, extendable to 1 year. There will be flexibility to tailor the job plan and clinical experience to the specific demands and skills of the successful candidates. 5) The post-holder will be expected to work in a professional and courteous manner with the multidisciplinary team.
