The Phoenix Practice in Hendon is seeking a Clinical Pharmacist to provide expert clinical medicine reviews and address public health needs within GP practices. The role involves reducing inappropriate polypharmacy, improving the quality and safety of prescribing, and managing practice formularies to enhance cost-effectiveness. The Clinical Pharmacist will work collaboratively with a multidisciplinary team to assess and treat patients, particularly those with chronic diseases, complex polypharmacy, and vulnerable populations, such as the elderly and individuals with learning disabilities. The position emphasises best practices in prescribing through audits, adherence to NICE guidelines, and proactive management of patients' medication needs.

Ideal candidates will possess a pharmacy degree, independent prescribing qualifications, and extensive post-graduate experience. Responsibilities also include leading shared care protocols, providing educational support to prescribers, and participating in clinical research. The Clinical Pharmacist will play a key role in fostering collaborative relationships both within the practice and with external healthcare providers to ensure a holistic approach to medication management and patient care.
