Ideal candidates will possess a pharmacy degree, independent prescribing qualifications, and extensive post-graduate experience. Responsibilities also include leading shared care protocols, providing educational support to prescribers, and participating in clinical research. The Clinical Pharmacist will play a key role in fostering collaborative relationships both within the practice and with external healthcare providers to ensure a holistic approach to medication management and patient care. The position combines expertise in pharmaceuticals with a focus on improving health outcomes and addressing healthcare inequalities within the community.
Provide expertise in clinical medicines review 1 and address public health and social needs of patients in GP practices
Reduce inappropriate poly-pharmacy and wasteful prescribing through clinical medication review
Increase quality and safety of prescribing through mechanisms such as audit and PDSA cycles
Manage practice formularies to improve the quality, safety and cost effectiveness of prescribing
Implement drug withdrawals and alerts e.g. MHRA aimed at improving medicines safety
Work with primary care professionals and patients to implement NICE and other evidence-based guidelines
1 This would be a level 3 clinical medication review as per the Room for review document 2002/03. Look at the patients full clinical condition, blood monitoring, interface care arrangements, social isolation etc.

CORE RESPONSIBILITIES
1. Working within the practice based team to take responsibility and undertake medication reviews particularly in high risk groups
2. Improve prescribing practice through educational support for all prescribers within the practice
3. You will work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.
4. Take responsibility for the care management of patient with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple comorbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism (through STOMP Stop Over Medication Programme).
5. Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the South West Barnet PCN practices and to help in tackling inequalities.
6. Lead on where changes in evidence require changes in prescribing across patient population e.g. where a drug is withdrawn or indications change
7. Liaise with colleagues in community pharmacy to align support for medicines adherence such as MUR and NMS
8. Support improvements in clinical care through practice based audit and implementing change
9. Prescribing advice to prescribers in practice e.g. temporary non availability of drugs
10. Ensure patient safety when they are transferred between care providers through reconciliation of prescribed medicines
11. Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists and anticoagulation.
QUALIFICATIONS & TRAINING FOR THIS JOB
NB: it is anticipated that the level of qualification held may vary according to the level of position and the components of the role being carried out
Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council
Minimum of 5 years post graduate experience in pharmacy, as demonstrated within a practice portfolio
Member of the Royal Pharmaceutical Society (RPS)
Member of or working towards (through foundation) RPS Faculty membership
Currently hold an independent prescribing qualification
Hold a postgraduate pharmacy qualification.
Have qualified from an accredited training pathway that equips the clinical pharmacists to be able to practice and prescribe safely and effectively in a primary care setting (currently the CPPE Clinical Pharmacist training pathways)
Has an awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice.
