The Trainee Advanced Clinical Practitioner - Frailty role at Mid Cheshire Hospitals NHS Foundation Trust in Crewe offers an exciting opportunity to support the expanding Frailty team, which will feature a new Frailty Unit and collaborate with community teams for a virtual ward approach. Ideal candidates should have a passion for frailty care and preferably experience with elderly patients, as they'll work within a multidisciplinary team to address the complex needs of this patient population. The position involves developing personalized care plans, maintaining effective communication between secondary and community care, and progressing clinical practice through assessment, planning, implementation, and evaluation of patient care.

In this full-time role, the Trainee Advanced Clinical Practitioner will engage in flexible working hours, including weekends, while utilizing evidence-based practices to facilitate patients' journeys through the healthcare system. Responsibilities also include providing leadership, contributing to the education of the multidisciplinary team, and participating in clinical audits and quality improvement projects. Candidates must demonstrate independent clinical judgment and collaborate effectively within various departments, ultimately striving to enhance the quality of care and support innovative service development within the Frailty unit.
