Old Swan Health Centre Group Practice in Liverpool is seeking a motivated and enthusiastic Practice Nurse to join their friendly team, which serves 9,300 patients. The ideal candidate will possess strong clinical and communication skills, demonstrate empathy, and deliver high-quality nursing care, particularly in chronic disease management. The role includes providing comprehensive assessments, screenings, immunisations, health education, and developing nurse-led clinics tailored to the needs of patients with various chronic conditions such as asthma, diabetes, and hypertension.

In addition to performing essential nursing duties such as cervical screenings and immunisations, the Practice Nurse will collaborate with the team to develop practice protocols, maintain clinical knowledge, and participate in ongoing training and clinical audits. The successful applicant should have excellent organisational abilities and contribute effectively within a team environment, always prioritizing patient care.

The successful candidate will have excellent clinical and communication skills, a caring nature who will provide and maintain a professional manner and high standard of nursing care.

We are looking for an experienced practice nurse who is confident in chronic disease management and puts patient care at the centre of what they do. The successful candidate will have excellent organisational skills and works well within a team.

The candidate will work as part of a team in developing primary care services for our Practice population, providing assessments, screening, treatment, health education advice including chronic disease management, cervical screening, immunisations and health promotion independently or within agreed protocols.

About us
KEY RESPONSIBILITIES
1. Providing assessment, screening and treatment services that are relevant to patients and their families.
2. Developing and organising with others, nurse-led clinics for chronic disease management programmes within the practices, in line with national and local policies:
Asthma
Coronary Heart Disease
Chronic Obstructive Pulmonary Disease
Diabetes
Heart Failure
Hypertension
Peripheral Vascular Disease
Stroke/TIA
3. Performing immunisation including baby and travel immunisations
4. Learning Disabilities health assessments
4. Cervical smears
5. Develop practice protocols and guidelines in conjunction with the GPs

6. Maintain clinical knowledge and identify training needs
7. Participate in shared learning and clinical audits
