The Therapy Discharge Practitioner role at Calderdale and Huddersfield NHS Foundation Trust is a key position focused on implementing the Home First Model for patient care, allowing medically optimised patients to return home for assessment rather than remaining in the hospital. The practitioner will serve as a single point of contact for patients and their families, facilitating safe and timely discharges in collaboration with a multi-disciplinary team. Responsibilities include assessing patients’ needs, coordinating referrals to other health services, and conducting equipment evaluations to enhance patient care.

In addition to clinical duties, the post will involve patient support during emotional and stressful times, effective communication, and documentation in compliance with trust policies. The role requires participation in training, service improvement projects, and maintaining professional standards, as well as the possibility of lone working in patient homes. The successful candidate will be expected to work a flexible schedule, including weekends and holidays, while developing their therapeutic skills through ongoing competency training.
