Hospice at Home an independent charitable organisation; is looking to recruit to a flexible part time Patient Co-ordinator and admin support worker on 15 hours per week. The post holder will be able to work flexibly to meet the needs of the Nursing Coordination Team.
The post holder will be responsible for accepting referrals to the nursing service, allocation of patient care and providing general admin support to the clinical teams. The post will also require the individual to communicate with a variety of Health and Social Care Professionals, patients/carers and members of the general public on a daily basis.
The post holder will act as a patient co-ordinator for the Hospice at Home nursing and support service which provides palliative care to patients, their relatives and carers. The co-ordinator will be responsible for accepting referrals, the allocation of staff, liaising with other health and social care professionals, patients relatives and carers and the general public.
The post holder will work closely with others who co-ordinate care. The post holder will also provide administrative support to the Hospice at Home team which includes Nurse Team Lead, Complementary Therapy, Family Support, Lymphoedema, and Occupational Therapy services.
