There will be the opportunity to develop skills in chronic disease management e.g. asthma, COPD, diabetes, hypertension, cardiovascular and renal conditions.
Birchwood Medical Practice is looking for a nurse to join our fab team!
The postholder will ideally be able to carry out venepuncture, wound management, immunisations, diagnostics such as ECGs, and cytology and chronic disease reviews.
However, we recognise there are so many transferable skills and would also welcome applications from community or secondary care nurses who would like to take the step to General Practice. We are particularly keen to hear from those with a background/expertise in either wound care, respiratory care or minor ailments/injuries.

We are flexible and looking for 15 to 25 hours over 2-3 days to include
EITHER
a Monday or a Friday (not both unless you want to!) We require a minimum of 1 x 7am session start time in the practice
OR
1 x late finish 8pm to support Improved Access requirements. We would be happy to support working from home for a late finish or early start if preferred once fully competent in role.
We are offering a golden handshake of up to £2K (DOE) to the successful candidate - terms and conditions will apply to this payment.

Patients are generally unwell when they attend the surgery so we require kind, caring and patient people in our team.
We use the EMIS Web clinical system. Experience with this is desirable but not essential.

- Assess, plan, develop, implement and evaluate programmes to promote health and well-being, and prevent adverse effects on health and well-being
- Implement and evaluate individual treatment plans for patients with a known long-term condition
- Identify, and manage as appropriate, treatment plans for patients at risk of developing a long-term condition
- Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
- Support patients to adopt health promotion strategies that encourage patients to live healthily, and apply principles of self-care
- Deliver opportunistic health promotion using opportunities such as new-patient medicals
- Provide information and advice on prescribed or over-the-counter medication on medication regimens, side effects and interactions
- Support patients to adopt health promotion strategies that promote patients to live healthily, and encourage principles of self-care
- Assess and care for patients presenting with uncomplicated wounds
- Support and advise women requesting information relating to family planning needs
- Support and manage health needs of women presenting for cervical cytology consultations
- Recognise, assess and refer patients presenting with mental health needs in accordance with the National Framework (NSF) for Mental Health
- Implement and participate in vaccination and immunisation programmes for both adults and children
- Advise, support and, where appropriate, administer vaccinations for patients travelling abroad
- Promote and deliver evidence-based care for patients presenting with aural conditions
- Use auditing to improve best practice
- Assist senior practitioners in providing minor-surgery sessions
