The Non-Clinical Care Coordinator position at Hurley Group in London is a one-year fixed-term contract aimed at facilitating the safe and timely discharge of patients affected by gambling harm. The role involves working closely with the Mental Health Nursing Team to support nurses and manage a small caseload of patients with varying levels of complexity. Key responsibilities include ensuring patients understand their post-discharge plans, coordinating with healthcare providers and external agencies, completing necessary documentation, and participating in discharge meetings to communicate and plan discharges effectively.

The Coordinator will act as a liaison between patients and third-sector services, ensuring that discharge procedures are appropriately followed and that patients receive the necessary ongoing care. The position requires collaboration with GPs, therapists, and multidisciplinary team members to deliver a high-quality, patient-focused service while also escalating any issues to the Mental Health Team Leader as needed.
