                                                    Newly Qualified Salaried GP (ARRS) with Hammersmith and Fulham Central PCN Limited in London

                                                     We are seeking newly qualified GPs who have not held substantive posts to work within Hammersmith and Fulham Central PCN on a fixed term contract.   This is a position for newly qualified GPs (within 2 years of CCT date & not having held a substantive post before) to work within the Hammersmith and Fulham Central PCN on a fixed term contract until 31.03.25. The post-holder will manage a caseload and deal with a wide range of health needs in a primary care setting, ensuring the highest standards of care for all registered and temporary patients. This post will require you to travel between different practices within the PCN as and when required and will require you to be registered with the General Medical Council (GMC) and hold a certificate of completion of training within two years. This role does NOT apply to General Medical Practitioners who are beyond the second anniversary of their certificate of completion of training. This post may require you to travel between different practices within the PCN in the borough of Hammersmith & Fulham as and when required and will require you to be registered with the General Medical Council (GMC) and hold a certificate of completion of training within two years. Primary Responsibilities   In accordance with the PCN/Practice timetable, as agreed, the post-holder will make him/her-self available to undertake a variety of duties including surgery consultations, telephone/video consultations and queries, visiting patients at home, checking and signing repeat prescriptions and dealing with queries, paperwork and correspondence in a timely fashion.   Making professional, autonomous decisions in relation to presenting problems, whether self-referred or referred from other health care workers within the practice or external organisations.   Assessing the health needs of patients with undifferentiated and undiagnosed problems.   Screening patients for disease risk factors and early signs of illness.   Contributing to QOF requirements and recording data appropriately.   Contributing to PCN requirements and recording data appropriately.   In consultation with patients and in line with current practice disease management protocols, developing care plans for health-related illnesses.   Screening patients for health education.   Admitting and discharging patients to and from the caseload and referring to other care providers as appropriate.   Recording clear and contemporaneous consultation notes to agreed standards.   Collecting data for audit purposes and contributing to the practice audit programme as required.   Compiling and issuing computer-generated acute and repeat prescriptions using ESP (avoiding physical or hand-written prescriptions whenever possible).   Prescribing in accordance with the accepted prescribing formulary (or generically) whenever this is clinically appropriate.   Take responsibility for some aspects of the clinical work within the team, for example: coordinating aspects of governance, QOF, enhanced services, CPD and keeping clinical protocols up to date. In general, the post holder will be expected to undertake all the normal duties and responsibilities associated with a GP working within primary care, as set by Royal College of General Practitioners, General Medical council, all other Royal colleges and councils applicable to the staff within the practice, Department of Health, Clinical Commissioning Groups and all other regulatory and standard setting organisations.  Collaborative Working Relationships:   Foster and maintain strong links with all services across the PCN and neighbouring networks.   Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships.   Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues, and other professionals, other NHS/private organisations e.g. ICBs).   Can recognise personal limitations and refer to more appropriate colleague(s) when necessary.   Liaises with other stakeholders as needed for the collective benefit of patients including but not limited to:  o Patients and their representatives.  o GP, nurses and other practice staff.   Other healthcare professionals including community pharmacists, pharmacy technicians, social prescribers, first contact physiotherapists, physicians associate and paramedics.   o Community pharmacists and support staff   o Other members of the medicines management (MM) team including pharmacists, technicians and dieticians   o Locality/GP prescribing lead   o Community nurses and other allied health professionals   o Hospital staff with responsibilities for prescribing and medicines optimisation   o Community nurses and other allied health professionals  Professional Development:   Work with your line manager to undertake continual personal and professional development, taking an active part in reviewing and developing the role and responsibilities.   Involved in one to one meetings with line manager monthly to discuss targets and outcomes achieved.   Adhere to organisational policies and procedures, including confidentiality, safeguarding, lone working, information governance, and health and safety.   Review yearly progress and develop clear plans to achieve results within priorities set by others.
