The role is part of the services single point of access and is primarily a home based role, workingautonomously but with support available when required. Key elements include managing referrals, bookingappointments as well as providing assessment, sign-posting, education and advice to clients, carers and otherhealth and social care professionals to ensure they are able to access the most appropriate support dependanton need.   Previous experience is welcome but not essential just a willingness to learn and respond well to feedback. We provide full induction training, regular coaching and the support and encouragement needed to get you off to the best start.
