Practice Nurse with Boughton Health Centre in Chester, Cheshire
An exciting opportunity has arisen for an enthusiastic and motivated Practice Nurse to join our vibrant, forward-thinking practice. The successful candidate will be an integral part of the nursing team and will be responsible for participating in the delivery of high-quality patient care. We welcome applications from both newly qualified Practice Nurses and those with wider NHS/Primary Care experience. As a Practice Nurse your main duties will include the following: Our Practice Nurse will help patients with a wide range of long-term conditions and provide high standards of clinical care. You will work alongside our internal nursing team along with other community teams to deliver a high quality service to patients. Our Practice Nurse will perform tasks prescribed and delegated by the GP Partners, working unsupervised to Patient Group Directions, agreed protocols, within an evidence base and within defined competencies. Our Practice Nurse will assess patients and implement any treatment plans or reviews at their own discretion. Report to the Lead Practice Nurse (or other senior members of the practice team) with any concerns/queries. About us Main Responsibilities To provide assessment, screening and treatment services which are relevant to patients and their families. To assist in the provision of general practitioner services. To ensure own actions are consistent with clinical governance systems, monitor and maintain the health, safety and security of self and others within the practice. To participate in the administrative and professional responsibilities of the practice nursing team: to assist in the implementation of policies, standards and guidelines for nursing care within the practice. Attend and participate in practice and staff meetings Infection Control support patients and colleagues in adopting sound infection control measures. Emergency Management perform regular checks on all equipment and drugs and ensure records are up to date. To participate in the research/audit within the practice as required. To liaise with health authority staff in hospital and community, social services staff and voluntary organisations. To provide an adequate service to patients, and provide access to specialist nurse/GPs. To maintain professional development by attending courses and reading. To attend study days after discussion with senior GPs within the practice to take account the needs of the practice. Expected to attend learning events as required by the practice and to take part in an annual Appraisal process. Maintain electronic patient records: input data at time of consultation; update knowledge of annual QOF requirements. Maintain confidentiality of patient information. You have a responsibility to follow the principles of safeguarding vulnerable adults and children in accordance with Practice Policy. General Duties: Act as a chaperone when needed Administer routine injections New Patient Medicals/Urinalysis Nursing of minor ailments Monitoring blood pressure and Hypertension Management Peak Flow / Spirometry Performing ECGs Phlebotomy and INR testing Requesting pathology tests: urine culture, swabs and blood tests Wound care Ulcer care and dressings Doppler Removal of Sutures and Clips Undertake Health Screening, Surveillance and Health Promotion Provide support to patients in changing behaviours/habits: diet, exercise, alcohol, smoking cessation Childhood immunisations Recognise signs, symptoms and categories of abuse, to be aware of safeguarding procedures, statutory local procedures and points of referral Chronic Disease Management (Including Diabetes, Asthma, COPD): Assist in maintaining GP contract regarding chronic disease patients Additional training for chronic disease management can be provided Work within practice guidelines, ensuring safe practice and acknowledge limitations Work alongside practice team to reach/achieve QOF targets This job description is not exhaustive and is considered as a guide only and will be subject to change in consultation with the post holder.
