Boughton Health Centre in Chester is seeking an enthusiastic Practice Nurse to join their dynamic nursing team. The role involves delivering high-quality patient care, particularly for those with long-term conditions, and requires the ability to work autonomously under established protocols and Patient Group Directions. Candidates can be newly qualified or have prior NHS/Primary Care experience.

Key responsibilities include conducting patient assessments, implementing treatment plans, maintaining clinical governance standards, managing patient safety, and participating in administrative duties. The Practice Nurse will also engage in health promotion, chronic disease management, and general support for both patients and the nursing team.
