The Community Nursing Single Point of Access is a contact centre for Community Nursing Services within NHFT including District Nursing, Intermediate Care and Specialist Palliative Care. The team is an integral service within Community Nursing.
The post holder will be working in an integrated skill mix team of both administrators and clinicians and will work closely with the Community Nursing teams. This is an important, customer focused role involving acting as an interface with patient and their families as well as healthcare professionals and external stakeholders who wish to refer to NHFT Community Nursing Services.

The role involves beingthe first point of contact for patients and health professionals who are calling to make contact with our Community Nursing Services; prioritising calls and addressing urgent needs using a clinical software programme whilst working closely with the clinicians.
The SPoA service currently runs 8-6pm, with potential bank shifts to be taken in shift patterns of8am-4pm and 10am-6pm.
