East Coast Community Healthcare CIC is seeking a motivated and adaptable Nursing Sister/Charge Nurse/District Nurse to join their Primary Care Home team in Lowestoft. This role involves providing high-quality nursing care, leading a multidisciplinary team, and managing a designated caseload of patients in the community. Responsibilities include conducting comprehensive assessments, formulating individualized care plans, and promoting patient independence. The successful candidate will also engage in HR functions, support junior staff, and ensure adherence to best practices and legal standards within community healthcare services.

The position is offered for up to 37.5 hours per week with a flexible rota including weekend and bank holiday shifts.
