The South West London and St George's Mental Health NHS Trust is seeking a dedicated Band 6 Social Worker to join the multi-disciplinary team on Hume Ward, a 15-bed male low secure unit in London. This role involves providing comprehensive social work services to forensic inpatients, which includes conducting detailed assessments, compiling social histories, and addressing safeguarding concerns for both adults and children. The social worker will collaborate with various professionals within the multidisciplinary team to manage patient care and facilitate timely transitions back to community settings, while ensuring compliance with relevant mental health legislation and local authority services.

The post holder will take an active role in care planning, discharge processes, and fostering communication among stakeholders, while promoting anti-discriminatory practices and equal opportunities. Ongoing professional development is encouraged, with responsibilities that include participating in therapy and social activities, conducting assessments for statutory duties, and preparing reports for mental health tribunals. The ideal candidate must possess a social work registration accredited by the British Association of Social Workers (BASW) and demonstrate effective communication and teamwork skills in a challenging clinical environment.
