The role of Adult Hospital Liaison Practitioner at Rotherham Doncaster and South Humber NHS Foundation Trust involves joining the Adult Mental Health Hospital Liaison Team at the Woodlands Unit, providing urgent and emergency responses to mental health referrals from The Rotherham Foundation Trust. The practitioner will conduct comprehensive bio-psychosocial assessments and collaborate with TRFT staff to create safe and supportive discharge plans, as well as enhance staff knowledge of mental health through training and supportive practices that prioritize patient-centered care.

The ideal candidate must possess strong assessment and risk assessment skills, demonstrate the ability to work effectively under pressure, and manage responsibilities both autonomously and as part of a multidisciplinary team. Candidates should have experience working with adults facing complex mental health challenges and a commitment to ongoing professional development. The position operates within a 24/7 framework, requiring a flexible full-time commitment.
