Alliance for Better Care are advertising on behalf of East Surrey Modality Medical Practice - Horley PCN, who have an exciting opportunity following the announcement of the new GP ARRS role. Healthy Horley PCN are looking for 2 newly qualified (within 2 years) GPs on a fixed-term salaried contract until 31st March 2026 (with the view to extension). Healthy Horley PCN are seeking highly motivated and dedicated newly qualified GPs to join their multidisciplinary team under the NHS Additional Roles Reimbursement Scheme (ARRS). This flexible position requires a commitment of 4 sessions a week and includes an opportunity for 1 of these sessions to develop an area of special interest in one of the following:

• Frailty / Anticipatory Care
• Lifestyle Medicine
• Urgent Care
• Womens Health

Healthy Horley PCN are a high performing and collaborative network of practices with a strong commitment towards integrated partnership working, neighbourhood development and continuity of patient care. We use EMIS and our most recent CQC rating was Good.

The successful post-holder will be employed by East Surrey Modality Medical Practice.

Healthy Horley PCN have an exciting opportunity to expand their Primary Care workforce by employing a newly qualified Salaried GP. The PCN deliver a wide range of services including a Children and Young Persons Wellbeing Service, Anticipatory Care / Frailty Hub, GP Integrated Mental Health Services, Employment Specialist Services and a Community Health Hub (Horley Health Hub). We are proactively engaged with the Neighbourhood Health and Wellbeing Networks and aim to remain at the forefront of community engagement and Integrated Neighbourhood Teams as they develop.

About us

Primary duties and areas of responsibility

The successful post-holder will:

• Provide a range of medical services as defined in the core GMS contract, additional and enhanced services where appropriate.
• Assess the healthcare needs of patients with undifferentiated and undiagnosed problems and make professional, autonomous decisions in relation to presenting problems.
• Provide face-to-face and virtual consultations for routine and urgent care to meet identified patients need.
• Conduct patient home visits (where required).
• Manage patient care within the Practice, including managing patient queries, investigating and reviewing results, developing care plans and making necessary referrals in a timely manner.
• Issue and sign computer generated acute and repeat prescriptions (avoiding hand written prescriptions whenever possible), adhering to national and local guidelines.
• Work with our clinical and support teams in delivering holistic and patient centred care.
• Maximise the effective use of current clinical computer systems.
• Record clear and contemporaneous consultation notes to an agreed standard using EMIS.
• Ensure safe, evidence based and cost effective care in line with Practice disease management protocols and NICE guidelines.
• Contribute to QOF & IIF requirements and recording data appropriately.

Health Promotion and Patient Support:

• Develop care plans for health in consultation with patients and in line with current practice and disease management protocols, provide counselling and health education.
• Screen patients for disease risk factors, lifestyle factors and early signs of illness and support patients in adopting health promotion strategies and self-care principles.
• Contribute to the delivery of anticipatory care planning.

Professional Development:

• Maintain awareness of clinical practice developments.
• Attend team meetings and other clinical meetings as necessary.
• Participate in performance reviews and take responsibility for your own personal development.
• Engage in mandatory training and attend courses as required.

Team Collaboration:

• Provide clinical support and guidance to both clinical and administrative staff to maintain the highest care standards and support others professional development.
• Work alongside the Practice Clinical Pharmacy team to review and manage patient medications following the appropriate policies and local clinical guidelines.
• Collaborate with health and social care providers to deliver an integrated, safe service.
• Actively participate in MDT and care conference meetings as required.
• Attend team meetings and other clinical meetings as necessary.

Risk Management and Compliance:

• Adhere to all practice policies and procedures, including those related to prescribing, confidentiality, and data protection.
• Work within current ICB policy for prescribing.
• Apply infection control measures and manage risks in compliance with local and national guidelines.
• Contribute to clinical governance through clinical audits, data collection, peer reviews and significant event analysis.

Patient Safety and Confidentiality:

• Ensure patient and carer confidentiality, in line with practice policies.
• Manage patient data securely, only disclosing to authorised persons.

The post-holder will be expected to undertake all the normal duties and responsibilities associated with a GP working in primary care, as set by the Royal College of General Practitioners, General Medical Council, all other Royal Colleges and Councils applicable to the staff within the Practice, Department of Health, ICB, CQC and all other regulatory and standard setting organisations.
