The Midlands Partnership NHS Foundation Trust is seeking a qualified Mental Health Practitioner to join their Primary Care Mental Health Practitioner Team in Lichfield, Staffordshire. The ideal candidate will have professional registration in nursing, occupational therapy, or social work and will be responsible for providing assessments, brief interventions, and support for individuals with mental health needs within a primary care setting. The role emphasises creating responsive and holistic mental health services, collaborating with primary care networks, and bridging gaps with other community services to ensure timely and effective care.

In this developing role, you will contribute to the implementation and review of care planning and risk management while promoting mental health awareness and fostering relationships between primary care and local communities. The position requires independent travel across a large area, and candidates must display a commitment to the organisation's core values and recovery principles. Experience or first-hand insight into the role is encouraged, with opportunities to shadow current practitioners available for interested candidates.
