                                                    Respiratory Clinical Nurse Specialist with University Hospitals Sussex NHS Foundation Trust (279) in Chichester

                                                     An exciting opportunity has arisen to join our passionate and dedicated Respiratory nursing team for a 12-month secondment. We are looking for a self-motivated and enthusiastic individual with previous experience of chronic respiratory disease management and a keen interest in further development.   You will need to have excellent communication skills, be able to work independently as well as part of a multidisciplinary team and be committed to high standards of care. You will demonstrate a high level of practice and knowledge participating in direct patient care and management of long-term conditions such as COPD, Asthma, bronchiectasis, interstitial lung disease and management of non-tuberculous mycobacterium infection. This will involve acute care supporting inpatient management plans, supported discharge, participation in hospital avoidance, pulmonary rehabilitation and independent nurse led outpatient clinics. You will be required to assist with the weekly pleural clinic alongside a Respiratory consultant. The role will need someone who is motivated, well organised and able to manage a caseload of patients. The respiratory Nursing service is currently delivered over 5 days a week, Monday-Friday.   You will join a team of Respiratory nurses and administrators and will be actively working within a multidisciplinary team of Nurses, consultants, physiotherapist and palliative care specialist.   The post holder will be a vital member of the St Richards Respiratory CNS team working with GP's, Practice nurses/nurse practitioners to support the screening, planning and monitoring of patients diagnosed with asthma, COPD, ILD and bronchiectasis. The successful post holder will be involved in managing a caseload of patients, and support patients to become independent with their management and treatments to deliver high quality care to patients with asthma & COPD, both on an inpatient and outpatient basis; assisting to identify those patients who may be eligible for biological asthma treatments. The post holder will support the assessment of patients suitability for pulmonary rehabilitation.   The post holder will be involved in education, audit and research and support the development of the severe asthma service. About us  Clinical     To lead best practice in nursing care of patients with asthma.Support patients referral onwards, including contributing to planning diagnostic work ups to support asthma/severe asthma diagnosis.   Completing and sending necessary information to the severe asthma MDT to support patient initiation on biologics.   The post holder will be able to assess, monitor and provide asthma action plans in the nurse led clinics.   The post holder will be able to perform spirometry.   Act as patients advocate and counsel when informed discussion may lead to choices being made concerning treatment options or quality of life issues.   To collaborate with other professionals and services in both secondary and primary care to achieve a seamless service, with effective communication between the service, regional hospitals and community teams.   Provide a telephone support service for patients and health care professionals, communicating with other services as indicated.   To act as an autonomous practitioner and provide nurse led clinics, request, perform tests.   Demonstrate nursing practices relevant to the area of asthma, ordering, performing and interpretation of diagnostics.   Ability to apply excellent analytical and judgement skills to interpret and compare a range of options on patient care.   Demonstrate ability to act independently, use initiative and be accountable for own actions in the hospital and outpatient setting.   Computer literate, able to use databases and other relevant programmes.   Demonstrate the ability to educate and train other health professionals, to motivate and enthuse others and work across professional boundaries.   Demonstrate excellent communication and interpersonal skills and show the ability to influence and negotiate.   Active participation in research and/ or clinical evaluation and audit.   Ability to present relevant written and visual educational material.   Flexibility, adaptability to meet the needs of a changing service.   To raise the profile of asthma in the trust, locally and nationally and actively participate in the promotion of asthma health.   To act in accordance with the NMC code of Professional standards of practice and behaviour for nurses, midwives and nursing associates.
