The Integrated Urgent Care Triage Nurse at Norfolk Community Health and Care NHS Trust in Norwich is responsible for triaging incoming patient referrals, assessing their needs, determining care priorities, and processing these referrals appropriately. This role involves collaboration with patients, their carers, and various clinical teams to ensure integrated and high-quality healthcare delivery. While primarily a non-patient facing position, the nurse will maintain clinical competencies and be involved in evaluating and managing care interventions across services, including the Single Point of Access, Urgent Community Response, and Virtual Ward.

Additionally, the nurse is expected to manage and document patient triage notes, escalate care concerns when necessary, and coordinate with multiple healthcare professionals to ensure comprehensive care. Proficiency in the SystmOne ledger management system and remote monitoring for Virtual Ward patients is essential, as is adherence to infection control policies. The position also requires flexibility to support clinical activity across different hubs, ensuring effective triage and allocation of responses within their scope of practice.
