The Assistant Practitioner/Nursing Associate for the Health Check Liaison Team at South London & Maudsley NHS Foundation Trust is responsible for enhancing the physical health of patients with severe mental illness across various London boroughs. This role involves independently running physical health clinics, providing outreach support to community mental health services, and facilitating access to appropriate health interventions. The Assistant Practitioner will also be tasked with documenting patient information in electronic records, producing reports, and collaborating with primary care services to share health outcomes.

Candidates for this position should possess strong relationship-building skills to effectively engage with patients and health professionals while managing their own workload in alignment with service goals. The role includes conducting essential physical health checks, such as monitoring BMI, blood pressure, and lifestyle habits, and requires adaptability as team assignments may vary based on demand.
