Our Clinical Decision Making (CDM) team are based within our Single Point of Access (SPoA) for Community services in Wye Valley NHS Trust. Our current team of clinical decision makers is growing to support the development of our community services.

The CDM supports the clinical triage and streaming of referrals in our Community Referral Hub. taking referral directly from clinical referrals where its more effective they speak directly with a clinician, Undertaking remote assessments over the phone as required and supporting patients in accessing the service they need but screening referral information as required.

The Community Referral Hub currently manages referrals and patient queries for the District Nurses, urgent response, virtual wards, Virtual GP, IVOPAT, Therapies (OT/PT) and care.

As well as liaising directly with patients and referrers the CDMs support our administrators and call handlers to ensure efficient and effective service delivery in the hub.

Our CDMs will screen referrals and decide which services they require - completing phone assessments to gather pertinent information as required.

Our CDMs will be proactive and approachable, supporting junior staff in gathering information and ensuring the seamless day to day running of the hub in support of the clinical leads and administration managers.
