The role of "FastTrack Discharge Co-ordinator" at Worthing Hospital, University Hospitals Sussex NHS Foundation Trust, involves working closely with the Discharge Team to facilitate timely discharges, particularly for end-of-life patients. Ideal candidates will have experience, but training will be provided. Key responsibilities include conducting Fastrack assessments, attending daily huddles and weekly MDT meetings, overseeing individual complex discharges, ensuring compliance with NHS guidelines, and educating other staff on discharge planning. The position also requires collaboration with community partners to enhance patient experience, reduce hospital stay lengths, and support the integrated Discharge Hub.
