The Family Support Practitioner at Hospice of the Valleys in Gwent is a part-time role (24 hours per week) aimed at providing essential welfare rights advice and support to patients and their families affected by palliative diagnoses. The practitioner will work within the Family Support Service, focusing on delivering high-quality, holistic care and support to individuals experiencing complex physical, emotional, social, and psychological symptoms. This includes assessing needs, creating care plans, providing bereavement support, and helping families navigate local services.

The ideal candidate should have at least two years of experience in a healthcare or social care setting, particularly with those impacted by life-limiting illnesses or bereavement. Responsibilities will include establishing professional relationships, facilitating communication, and contributing to training programs for hospice staff and the wider community. A sound understanding of local resources and effective communication skills are essential, as the role involves working autonomously within a multidisciplinary team to empower patients and families while promoting their independence and well-being.
