MULTI-DISCIPLINARY NHS TRANSPLANT
The University Hospitals Plymouth NHS Trust is seeking a full-time Pathway Co-ordinator for the Renal and Transplantation Department at Derriford Hospital. The role involves providing comprehensive administrative support for patients on the transplant waiting list and ensuring efficient follow-up care. Key responsibilities include coordinating multi-disciplinary meetings, managing communication with various stakeholders, and supporting the live donor program. The ideal candidate should demonstrate strong communication skills, a professional attitude, and the ability to manage conflicting priorities. Preference will be given to internal Trust staff and those with relevant experience.

The Pathway Co-ordinator will assist in enhancing patient care pathways, maintain accurate patient records, and support the clinical team's operational needs. This position requires a high level of integrity, the ability to work under pressure, and collaboration with other departments and healthcare providers. The role may also involve mentorship of junior staff and cover for colleagues during peak times or absences. Additionally, candidates should be prepared to deal with emotionally challenging situations that may arise in the context of providing care to patients.
