The All-Age Continuing Care CYP Transition Case Manager role at NHS Lincolnshire Integrated Care Board in Sleaford seeks a registered children's or learning disability nurse with experience in caring for children and young people. This position, which operates Monday to Friday with no required work on Bank Holidays, focuses on improving health outcomes for individuals with complex health needs, including learning disabilities and autism. Key responsibilities include conducting comprehensive assessments for NHS Continuing Care eligibility, supporting young people and their families during the transition to adult services, managing care packages, and collaborating with multidisciplinary teams and external agencies to ensure high-quality care.

Candidates should be proactive, resilient, and possess strong clinical, communication, and assessment skills. The role involves working autonomously, promoting Personal Health Budgets, representing the All-age Continuing Care team in interagency meetings, and maintaining a comprehensive database for individuals receiving care.
