Nottingham CityCare Partnership CIC is seeking an enthusiastic and motivated Occupational Therapist to join their community beds team. This role involves assessing and rehabilitating patients who cannot be discharged from the hospital due to care requirements, as well as providing support to prevent hospital admissions. The successful candidate will work within a multi-disciplinary team, conducting holistic assessments, developing goal-focused rehabilitation programs, and ensuring safe discharge plans. This position offers the opportunity to enhance clinical skills through a supportive Preceptorship Programme and provide training for other staff members.

The Occupational Therapist will be responsible for a defined caseload, working closely with senior colleagues and other healthcare professionals to improve patient outcomes. Key responsibilities include comprehensive client assessments, formulating individualized treatment plans, advocating for clients, and maintaining high standards of clinical practice. The role requires effective collaboration with partner organisations and a commitment to professional development and evidence-based practice. Additionally, the therapist will participate in clinical governance and help promote the role of occupational therapy within the healthcare team.
