Main duties of the job - This post will form part of the core adult clinical service, being integrated into the adult cardiac team where you will assist with the day-to-day service.
- Taking detailed medication histories, prioritising and actioning medication orders from ward staff, and providing medication counselling at discharge.
- To provide support and input to the clinical pharmacy team, including to the continuing development of the pharmacy service. There is an opportunity to gain experience in performing appraisals and supporting junior staff.
- To manage time and resources, prioritise daily tasks depending on workload, staffing and the needs of the service to ensure that pharmacy services are provided in a timely and appropriate manner at all levels within the hospital.
- To obtain and verify patient's drug histories in order to ensure the accurate continuation of patient's medication from Primary to Secondary care.
- To assist the clinical pharmacists, medical and nursing staff in identifying, prioritising and resolving patient's medication related problems.
- To minimise risk to patients, the public and other healthcare professionals by giving appropriate advice on drug handling, administration and disposal.
