This is an exciting opportunity for an experienced Care Co-ordinator to join our specialist service for people aged 14-35 experiencing a first episode of psychosis. The successful candidate will work collaboratively with service user, loved ones and will work within the multi disciplinary team to ensure effective care coordination.
To ensure effective care coordination and delivery of a nice concordant care package within an assertive outreach model.
