As a Connecting Communities Support Worker at HMP Liverpool, you will engage with individuals in custody who utilise our services, assisting them in preparing for a successful transition back into the community.

Hours 37.5hrs per week

Salary £23,400.00 - £24,466.99 Pro Rata depending on experience

This is a Fixed Term Contract until 31/03/2025

Role Overview

  • Hold caseload of complex case patients due to release, referred by Prison Healthcare Teams.
  • Be required to work with key prison stakeholders enabling effective coordination and transfer of service user treatment.
  • Work in partnership with other healthcare providers as part of an Integrated Health and Wellbeing model.


Key Responsibilities

  • Work intensively with patients for 12 weeks prior to and post-release.
  • Develop relationship with patients and co-design a holistic resettlement plan that ensures continuity of care for all their healthcare needs.
  • Review personal goals, preparing patients for release (e.g. what to expect on day one of leaving the gate).
  • Work closely with CGL Teams, Healthcare, Prison partners and External Support Agencies to ensure effective coordination of Release Planning.
  • Contribute to risk management processes, including prison ACCT process as appropriate.
  • Provide accurate and up to date harm reduction advice, including delivering training clients in the use of Naloxone.
