Torbay Carers Services is a small, dynamic team working hard to identify and support unpaid Carers across Torbay.
The role will identify Carers in Torbay Hospital and surrounding community hospitals and provide them with signposting to support while the person they care for is staying on the wards.

The post holder will identify Carers of patients on the hospital wards using personal contact with appropriate staff and using hospital systems to gather contact information for Carers. They will call Carers and signpost them to appropriate support, including registering Carers to Torbay Carers or Devon Carers.

  • Identifying Carers through personal contact with ward-based staff and using established systems.
  • Calling Carers to understand their situation and signpost to appropriate support, including that provided through Carers Services.
  • Recording contacts efficiently and reporting outcomes.
  • Linking with key colleagues to build relationships and improve Carer identification methods.
  • Raising concerns for Carer wellbeing and identifying where referrals need to go to other professionals for timely support.
