An exciting opportunity has arisen at Six Physio, where we are looking for an experienced Senior Domiciliary Physiotherapist to join our team: caring for patients in their community and within residential care homes, across Central, South-West and North-West London. Chelsea Home Physio joined Six Physio in Jan '24, founded 12 years ago they bring their wealth of experience with a reputation that precedes them, so we're now expanding our offering!
ROLE AND RESPONSIBILITIES
- Reviewing patients' medical history and relevant operation notes
- Developing a personalised treatment plan for each patient, outlining their goals and expected outcomes. Consulting with colleagues, family members and other members of the MDT
- Planning, implementing and managing patients' rehab plans
- Recording and evaluating patients 'progress on TM3 system
- Complying with HCPC and CSP regulations, standards and code of conduct
- Providing reports to Insurance companies, colleagues as required
- Exceptional communication and interpersonal skills
- Excellent time management
- In depth knowledge of neurological and orthopaedic conditions and treatments, and aged care/rehabilitation
- Neurological skills and experience
- Orthopaedic skills and knowledge
- Hydrotherapy experience desirable
- Experience of working with the elderly and patients with dementia
- BSc or Equivalent in Physiotherapy
- Up to date record of CPD
The successful applicant must have experience of lone working and managing travel between patients.
An to date DBS certificate is preferred.
