An exciting opportunity has arisen in the Bolton Community IV therapy team within the Integrated Community Division. The role is that of a band 6 staff nurse.This is a full time post and will be required to work between the hours of 8.30am and 10 pm, across 7 days a week.The post holder will work as part of the IV Therapy team with the supervision of the IV Team Leader to assist in the prevention of inappropriate hospital admissions and the facilitation of hospital discharge in line with the overall objectives of the service.
You will be accountable for the delivery of high quality care to patients within their own home and other community settings. Nursing interventions include Home blood transfusions, Administration of intravenous antibiotics and chemotherapy in a domiciliary setting as well as the delivery of IV medication in a clinic setting. The team work closely with Rheumatology, Gastroenterology and haematology specialist in treating a wide range of conditions.You will support with the monitoring of short term treatment programmes at home and ensure safe discharge or referral to long term health and social care interventions. The post holder will promote health/wellbeing and independence for patients on the team caseload to ensure a safe and effective IV service is delivered. Assessing patients suitability for OPAT.
The postholder will co-ordinate the shift, managing workload, capacity and demand.
