Job summaryWe currently have a fantastic opportunity for a clinical professional to join the CHC team in the ICB. The ICB is looking for a registered nurse, occupational therapist or physiotherapist with experience of clinical requirements of one to one in care homes for people with dementia or other health needs. The ideal candidate will be experienced in identifying the least restrictive options for care and the alternatives to support care homes in mitigating risks to their residents from falls or behaviours that challenge. The role will provide an opportunity to make a real difference to people receiving care in care homes whilst supporting the quality projects for the ICB in year plan.
Main duties of the job - Providing expert support and advice to homes

the post holder will be looking to work closely with identified homes to consider alternative to the one-to-one care. Working to support the home to manage risks both from falls and behaviours that challenge. - to work with the CHC transformation manager to utilise all the information gathered to maintain the ongoing log of existing one to one requests, reviews and next review dates and to ensure reviews are completed in a timely manner.
- To facilitate one to one panels to provide challenge and support to case managers in regards to one to one care requests.
