An exciting opportunity has arisen for a dynamic, forward-thinking band 7 highly specialist Occupational Therapist to join our Place Based Care Team in Newport.
We are making excellent progress in Newport with developing our Place Based Care (PBC) model to ensure patient centred Primary and Community Care is delivered as close to home as is possible to supports people's health and wellbeing whilst promoting prevention and self-management opportunities.
You will work alongside the Clinical Lead Nurses for Place Based Care and collectively provide support to our established team of MDT Coordinators to work proactively GP Practices and multi-sector partners including via the Virtual Ward /MDT meetings to support and prevent deterioration for patients living with frailty, complex illness and emerging biopsychosocial needs.
Utilising your experience and skills you will deliver Occupational Therapy to support a range of early proactive multi-professional interventions that avoid health exacerbations, deterioration and presentations to the GP/OOH/UPC/ED for non-medical assistance.
