Band 4: £31,944 to £34,937 a year per annum inclusive of HCAs  FIXED TERM 
To work autonomously within structured boundaries which are risk assessed and supervised by a registered healthcare professional.

  • To work within a multi-professional team and provide support to a team of health care professionals
  • To communicate effectively with patients and carers, using motivational interviewing skills where necessary.

  • To ensure practice is evidence based, by keeping up to date with new research literature, and by regularly evaluating working practices through evidence-based practice projects, audit, and research.
  • To demonstrate compassion, empathy and sensitivity to clients who may have complex emotional and physical conditions and social concerns requiring moderate emotional effort daily.

An exciting opportunity has arisen for a Band 4 Community Health Care Support Worker to join our Intermediate Diabetes Specialist Service (IDSS) and Heart failure Specialist Service at Whittington Health. This is a joint working post for a health inequalities project. The project involves supporting patients who may face challenges engaging with healthcare professionals specialising in Diabetes and Heart failure. Within this role the post holder will support registered healthcare professionals to deliver diabetes and Heart failure care. There will be elements of patient contact, ensuring that the administration involved in care delivery is completed and supporting in facilitating some group consultations as delegated by the Specialist teams.

  • To work within a multi-professional team and provide support to a team of health care professionals
  • To support specialist nurses in clinic settings and on home support/case management which may involve domiciliary visits to patient's homes.
  • To be responsible for administrative support within the team as required.
