Role
As a Social Worker in Adults Services & Health, you will be responsible for assessing the needs of adults, developing care plans, and ensuring that individuals receive the appropriate support and services. You will work closely with a multidisciplinary team to promote the well-being and independence of our clients.
Key Responsibilities
Conduct comprehensive assessments of adults' needs.
Develop, implement, and review personalized care plans.
Manage a diverse and complex caseload.
Collaborate with healthcare providers, families, and other stakeholders.
Advocate for the rights and needs of adults in the community.
Qualifications
Qualified Social Worker with relevant experience in adult services.
Strong assessment and care planning skills.
Excellent communication and teamwork abilities.
Ability to manage complex cases effectively.
