Operational
Conduct comprehensive assessments of individuals' healthcare needs, functional abilities, and support requirements. This involves reviewing medical records, conducting interviews, and consulting with healthcare professionals involved in the individual's care. You will evaluate the complexity and intensity of their needs to determine if they meet the eligibility criteria for CHC funding
Based on assessment findings, make informed decisions regarding the eligibility of individuals for CHC funding. This involves critically analysing the evidence, applying relevant policies and guidelines, and using professional judgment to determine the appropriate funding pathway for each individual.
Use clinical expertise to the assessment process. This includes having a sound understanding of various medical conditions, healthcare interventions, and treatment options. Using clinical knowledge to assess the suitability of different care settings and services for individuals.
Work closely with multidisciplinary healthcare teams, social workers, and other professionals involved in the CHC process. Effective collaboration and communication are crucial to gather comprehensive information, share assessment findings, and ensure that everyone involved has a clear understanding of the eligibility decision-making process.
Be responsible for providing guidance, training, and mentoring to junior assessors, ensuring they understand the assessment process, eligibility criteria, and documentation requirements. This supports maintaining consistency and quality in the assessment process across the organization.
Engage in continuous professional development, attend relevant training sessions, and actively seek opportunities to enhance ones knowledge and skills as a Senior Clinical Assessor.
Contribute to the quality assurance process by participating in audits, quality reviews, and feedback mechanisms. Supporting identify areas for improvement in the assessment process and ensures that decisions align with established standards and regulations.
Engage with service users, their families, and advocacy groups to gather feedback, address concerns, and communicate CHC processes and decisions. Building meaningful relationships with stakeholders promotes transparency, increases understanding, and ensures that the individual's voice is heard throughout the assessment process
Information Management
Accurately document assessment findings, eligibility decisions, and justifications in a clear and concise manner. Maintaining thorough records is essential for transparency, accountability, and continuity of care.
Planning and Organisation
To act as a resource for CHC advice and expertise to Health and Social Care staff with respect to CHC and FNC.
Policy and Service Development
Adhere to legal and regulatory requirements and ensure compliance with national frameworks and local policies. This includes maintaining confidentiality, following data protection protocols, and upholding professional and ethical standards.
Work within all statutory and organisational requirements to review patients against the eligibility criteria
Keep abreast of up-to-date with changes in policies, guidelines, and best practices in CHC
NHS Continuing Healthcare (NHS CHC) is a package of care for adults aged 18 or over which is arranged and funded solely by the NHS. North West London Integrated Commissioning Board (ICB) are looking for individuals to join the service in a variety of roles and bands. The NWL team has come together under a new structure and this is an exciting opportunity to join a dynamic and positive team.
North West London Integrated Care Board is looking to appoint a part time Senior Case Manager 0.6 wte; the post holder will report to the Clinical Lead for adults. The post holder is required to have a sound track record of delivering daily operational services ideally from an acute and or community background, additional experience of care homes and domiciliary care would be beneficial to the role.
The post holder will:
Be responsible for leading on the key operational day-to-day areas of work relating to CHC for people. Ensuring that the CHC service is fully compliant with and following the national frameworks.
Providing a duty role to the service on a rotational basis within the team
Holding a CHC complex case load for adults who are CHC eligible
Support joint working with the LA ICBs and clinical teams within North West London
As an experienced nurse or AHP you will have a working knowledge of Mental Capacity Act, Safeguarding, experience of undertaking assessments, case and risk management and personalised care agenda will be essential for this role
We are looking for innovative, forward thinking, enthusiastic and confident nurses or AHPs to be part of the skilled and dynamic team. Previous experience of working in NHS Continuing Healthcare would be an advantage and a level of knowledge and experience of the framework is required.
The post-holder will undertake various functions relating to the National Framework for NHS Continuing Healthcare which will include undertaking health needs assessments, facilitation of multi-disciplinary assessments, reviews, supporting individuals with case management and staff leadership and management.
You will be an excellent communicator with sound interpersonal skills. You will need to have the ability to manage emotive and sensitive issues with tact and diplomacy, work well under pressure, negotiate with confidence, and maintain a high level of productivity. You will become part of a diverse multi-skilled team, who support each other with their own clinical knowledge to ensure the best personalised care for each individual client.
You will complete all aspects of NHS Continuing Healthcare and strive to improve processes and services. You will respond and report safeguarding concerns working closely with system partners. You will ensure that DoLS compliance is followed. You will be required to demonstrate time management skills in your ability to manage a caseload, and work in a flexible manner within a pressurized environment.
