Job summaryTo be a key member of the Diabetes team structure providing specialist clinical management to patients with diabetes using specialist skills and knowledge to facilitate the effective management of patients with diabetes. The outcome of this activity is to avoid unnecessary hospital admissions via more effective management and prevention of acute exacerbations and the offer of early supported discharge to patients admitted to secondary care.
Main duties of the job - To work as an accountable and autonomous practitioner, by leading in provision of diabetes care for patients, within Lincolnshire. This is to include clinical care, education and training for patients and staff within LCHS.

- Improve clinical outcomes for these diabetes patients by enabling them to function independently by increasing their choice to remain in their own home/community and reduce the need for, or prevent inappropriate hospital admission.

- To be responsible for performance and target levels as identified by LCHS and the diversions
- Identification of very high intensity users with complex needs using risk stratification systems and other sources of information
- To support the Diabetes team to meet agreed quality and performance targets.
- Take a Leadership role in the continuous development of the service.
