We deliver acute hospital, primary, community and mental health care services to a population of over 1.15 million and a wider population of 2.2 million when our regional and national services are included.This post may close early to the volume of response. Shift pattern is 5/7 and the post holder will participate in the weekend and public holiday rota.
The post holder will
- support the provision of an accessible, appropriate, high quality, culturally sensitive and effective nursing service to all age groups in the community, by working in partnership with service users, care providers and other agencies.
- support the district nursing team to provide high quality nursing service.
- will be a HCSW who will support the district nurse in delivering care to patients within the caseload.
- will be responsible for providing support during skilled nursing interventions for patients within the caseload.
- will manage delegated activity within the caseload, and provide support to other members of district nursing team
