The following are the core responsibilities of the Advanced Clinical Practitioner.
a. The delivery of highly effective medical care to the entitled population
b. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice to patients.
c. Implement and evaluate individual specialised treatment plans for chronic disease patients
d. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on the patients health.
e. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
f. Request pathology services as necessary
g. Process and interpret pathology and other test results as required
h. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required
i. Maintain accurate clinical records in conjunction with extant legislation
j. Ensure read codes are used effectively
k. Maintain chronic disease registers
l. Prioritise health issues and intervene appropriately.
m. Support the team in dealing with clinical emergencies
n. Recognise, assess and refer patients presenting with mental health needs.
o. Extended and supplementary prescriber, adhering to extant guidance
p. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
q. Contribute to practice targets (QOF etc.), complying with local and regional guidance
r. Accept delegated responsibility for a specific area (or areas) of the QOF
s. Attend and contribute effectively to practice meetings as required
t. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
u. Support the clinical team with all safeguarding matters, in accordance with local and national policies
v. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
w. Prepare and complete the revalidation process
x. Commit to self-learning and instil an ethos of continuing professional development across the practice team

We are looking for an enthusiastic, motivated and forward-thinking individual who can work well as an autonomous practitioner or part of a multidisciplinary team.
To work as an autonomous practitioner, working as part of the practice multidisciplinary team, delivering care within their scope to the practice population. The post holder will be responsible for clinical areas such as, consulting patients with acute medical problems, health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.
