We are looking for a dedicated and organised Care Coordinator to join our team, as we expand and grow in new areas.
Responsibilities
Administration:
Manage and authorise staff rotas, ensuring accuracy and compliance with organisational procedures.
Maintain accurate and up-to-date records in line with regulatory standards and company policies.
Clients
Evaluate and process referrals for new care packages promptly and efficiently.
Coordinate care worker assignments in accordance with organisational standards to ensure high-quality service delivery.
Manage complex staff rotas to ensure adequate coverage and respond promptly to emergencies.
Liaison
Maintain effective communication with clients, families, and other relevant parties to ensure that service delivery meets expectations.
Miscellaneous
Attend meetings, briefings, and training sessions as required to stay informed and enhance professional skills.
Quality Monitoring
Assist in quality monitoring processes and maintain comprehensive health and safety records.
Monitor the quality-of-service delivery and contribute to comprehensive quality assurance measures.
Collaborate with the team to uphold and improve standards of excellence in practice.
Ensure compliance with all regulatory and company policies and procedures.
Staff Supervision And Development
Organise and oversee efficient work programmes, supervising staff to meet service objectives.
Participate in the recruitment process and provide necessary training to new employees.
Identify and address the developmental needs of the team to maintain high standards of care.
Coordinate and schedule care support worker visits to ensure consistent and timely service delivery.
What You Need
Previous experience in a care coordination or similar role within the domiciliary care sector.
Excellent organisational and time-management skills.
Strong communication and interpersonal skills, with the ability to build relationships with clients, families, and caregivers.
Compassionate and empathetic approach to client care.
Ability to work independently and as part of a team.
Proficiency in using care management software and Microsoft Office Suite.
Knowledge of relevant legislation and standards in domiciliary care.
NVQ Level 3 in Health and Social Care or equivalent qualification (preferred).
A clean and valid UK drivers license and access to a reliable vehicle.
