An exciting opportunity has arisen within our Urgent Community Response/Virtual ward Service for an experienced leader with a proven track record of operational and clinical experience to join the management team.
Our UCR/VW is an evolving service delivering acute assessment, management and monitoring within the community setting.
The post holder must have a thirst for clinical and operational leadership and be able to identify and drive change. If you feel you have these qualities then we would like to hear from you.

To support the evolvement of our Urgent community response/Virtual Ward service that leads on pathway development that will capitalise our 2 hour urgent community response target, virtual ward capacity and our new offer of tech enabled care in a patient’s own home.
To develop/expand the Step Up/Step Down offer of a Virtual ward and tech enabled care solutions.
To lead and evaluate test of change and to utilise QI methodology in driving the pathway development and service provision.
To support the Directorate Manager in developing the operational direction of the pathway services.
Actively support the Organisations Values and beliefs and share responsibility for the provision of the Pathway model services.
Work collaboratively with colleagues and managers to develop partnerships within the programme ensuring a co-ordinated approach in the delivery of the services/pathways.
To work collaboratively with multi agencies and ensure that all pathways adopt the health/social care cohesive partnership wherever possible and work in collaboration with the Clinical leads in relation to the quality agenda.
Along with Directorate Manager and Operational Lead offer management and leadership within the pathway model and ensure that clinical and operational performance is optimised.
Support Directorate Manger and Operational Lead in ensuring all performance KPIs are maintained and optimised.
