The following are the core responsibilities of the diabetic practice nurse. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
1. Provide a first point of contact within the practice for the management of diabetic patients
2. Initiate insulin and proactively monitor the care of all diabetic patients in line with local and national policy.
3. Provide home visits for those who are unable to attend the surgery
4. Assess the needs of patients ensuring the provision of healthcare is appropriate, incorporating evidence-based practice
5. Request pathology services and process pathology results as required
6. Develop, implement and embed health promotion and well-being programmes
7. Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
8. Request pathology services as necessary
9. Process pathology results as required
10. Deliver patient care as necessary, referring patients to secondary / specialist care as required
11. Maintain accurate clinical records using ARDENS templates
12. Ensure SNOMED codes are used effectively
13. Maintain chronic disease registers
14. Prioritise health issues and intervene appropriately
15. Support the team in dealing with clinical emergencies
16. Recognise, assess and refer patients presenting with mental health needs
17. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice)
18. Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
19. Support the clinical team with all safeguarding matters, in accordance with local and national policies
20. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
21. Deliver opportunistic health promotion where appropriate
22. Prescribe medication for patients in accordance with PGDs and own scope of practice
23. Review patient medications to enhance compliance
24. Comply with infection control protocol
We are looking for a candidate who has completed a nationally recognised diabetes education programme at postgraduate diploma level to work in the practice 2 days a week covering 14 hours a week
The role will include the management of diabetes including insulin initiation, health education, reviewing blood results, medication monitoring, and dose adjustments. The ideal candidate will have a passion for diabetes and delivery high quality patient care.
We are flexible and will work with the candidate to agree the days worked within the practice to cover 14 hours a week.
