- To Provide District Nursing and Community Care Services within the context of our primary care networks across the borough delivering integrated person centred responsive coordinated and high quality care services.

- To be accountable as a professional for actions in relation to own and work delegated to others acting at all times within the NMC code of professional conduct.
- To be aware of the multiple needs of service users and carers acquiring knowledge of services available to fulfil these needs and teaching these methods of care to carers where appropriate.
- To ensure the continuity of care by effective liaison with colleagues in the Integrated Long Term or intermediate short term Care teams members of the Primary Health Care team and the hospital setting.
- To participate in and develop health promotion activities for service users and families in line with Your Health cares and local community priorities.
- To contribute to service development.
- To participate in Clinical Governance activities such as clinical audit and research.
- To comply with current Evidence Based Guidelines Your Healthcare and relevant Royal Borough of Kingston policies and procedures.
- Undertake performance review and appraisals in accordance with Your Healthcares policy and procedure.
Communications and Working Relationships
- To provide leadership and direction to the nursing team in the absence of the District Nurse.
- To allocate work and objectively assess the performance of team members against clear agreed criteria.
- Maintains up to date and accurate records ensuring confidentiality is maintained in accordance with Your Healthcare RBK NMC guidelines and the Data Protection Act.
- To participate fully in the use of information technology collecting and entering statistical data relevant to service area. Support use of remote devices and mobile working within parameters of community nursing care.
- To participate in team meetings and facilitate or attend organisational meetings as required.
- To participate in and contribute to the development of the Integrated Care Team with other professionals in Primary and Social Care sectors.
- To facilitate a learning environment for pre registration nursing students and students from other health and social care settings completing reports and assessment documentation as required.
- In partnership with the District Nurse provide leadership mentorship and participate in the teaching and clinical supervision and training for Health Social Care and other staff and participates in the induction of new team members.
- Maintains professional competence and personal development by participation in mandatory and essential training which are reflected in the personal development plan.

- Dexterity in fine motor skills. Examples may include Intravenous procedures Syringe pump Male or Female and supra pubic catheterisations Topical Negative Pressure TNP and injectable implants.

We currently have a vacancy for enthusiastic and flexible Band 6 senior community nurse to join our community nursing teams in the borough of Kingston.
Rated OUTSTANDING for CARE by the CQC you will be supporting the team leader and working with a team of highly skilled and compassionate nurses nurse associates and health care assistants.
You will work closely with the team lead to assist with managing a caseload of patients, attached to one or two GP surgeries including case management of patients identified as being at risk of hospital admissions. You will act as a role model providing high quality care and skilled assessment of clients needs. You will also support team members developing their skills and providing feedback through 1-1 meetings and appraisal.
You will need to have 2 years experience at Band 5 in a community setting and must be a Practice Assessor. You will have experience of managing patients with a range of long-term conditions and multi co morbidities. Physical assessment and prescribing qualification are desirable.
You will work collaboratively with the wider multi disciplinary team other health and social care practitioners to provide the best care and quality outcomes for patients and service users. There will be opportunities for further development within the role.
You will need to be a car driver and have access to a vehicle. The position is full time worked over various shifts including weekends. Part time and job share will be considered.
