Exciting opportunity for a Band 6 Sister/Charge Nurse!

Working within our friendly District Nursing team in Wokingham, your role as a Band 6 Sister/Charge Nurse, will be pivotal within the Community Nursing teams to manage a nursing team and a caseload ensuring we deliver a seamless service to patients.
The Community Nursing teams are paramount in preventing avoidable hospital admissions and facilitating appropriate earlier discharges into the community.
- To operate within a Case Management framework and coordinate care for an identified caseload of patients.
- To be responsible for assessing patient need and to plan, implement and evaluate programmes of Nursing Care.
- To identify and plan preventative measures and anticipatory care needs.
- To proactively monitor patients following the review dates set out in the care plans.
- To assess patients with complex clinical need using clinical examination skills, history taking and evaluation skills
- To manage End of Life Care and work in partnership with health, social and voluntary agencies to ensure patients are treated with dignity and respect and to support their families and carers.
- To prepare the patient and their family/carer for changes in the patients’ condition and support choice for end of life care.
- To complete Management 1-1’s with your team as well as appraisals and ensuring staff are attending clinical supervision
- To submit statistical and contractual monitoring returns within agreed timescales.
- To be responsible for ensuring Mandatory and Statutory training is booked and completed within time scales of yourself and your team.
- To be responsible for booking and taking annual leave as laid down in Trust policy and Standard Operating Procedures and ensuring continuity or service by ensuring your team also take regular annual leave.
