1) Providing assessment, screening and treatment services that are relevant to patients and their families, with emphasis on improving diet and exercise routines through health promotion clinics. Referral to GP and other agencies as practice policy dictates.
2) Developing and organising, with others, nurse-led clinics for the chronic disease management programmes within the practice, specifically:
Coronary heart disease
Chronic obstructive pulmonary disease
Asthma
Diabetes
Heart failure
Peripheral Vascular disease
Hypertension
Stroke/TIA
3) Responsible, with other members of our nursing team, for the development, organisation and running of routine clinics specifically:
Immunisations
Travel health and vaccination programmes
Cervical smears
Contraception advice
4) Develop, organise and run the Practice immunisation programme.
5) Assess risk factors leading to ill health and advise and assist in suitable interventions.
7) Perform various clinical procedures such as ECG recordings, venepuncture, cervical smears, routine blood pressure monitoring, intramuscular and subcutaneous injections.
8) Devise self-management plans for patients with Asthma.
Administrative
1) Develop practice protocols and guidelines in conjunction with GPs.
2) Provide clinical advice and mentoring to less experienced clinical staff.
The Lakes Medical Centre is looking to recruit a motivated and experienced Practice Nurse to join our friendly and supportive team based in the Salford area. We have 6 strong GP team which also includes an Advanced Clinical Practitioner, 3 Practice Nurses,1 Physician Associate, 1 Health Care Assistant, Practice Pharmacist and First contact musculoskeletal practitioner who work closely to deliver high quality care to our 10,800+ patient population. Our premises also house Community mental health and Social prescribing clinics. We practise from purpose-built premises with easy access to Manchester city centre and the North West motorway network. As an established training practice, we are proud to have trained GP trainees, Advanced Nurse Practitioners and Foundation year doctors.

We are looking for an experienced practice nurse who is confident in chronic disease management and puts patient care at the centre of what they do. The successful candidate will have excellent organisational skills and works well within a team.
The candidate will work as part of a team in developing primary care services for our Practice population, providing assessment, screening, treatment services and health education advice including chronic disease management, cervical screening, coronary heart disease screening, well woman/well man clinics, immunisations, travel health, health promotion independently or within agreed protocols and in line with current evidence based practice.
Full UK driving licence is essential for the successful candidate.
