Our Therapists support hospital discharges, admission avoidance and long term self-management, enabling patients to live better lives in their home environment. We also provide Planned (Domiciliary) therapy to ensure patients are assessed and supported early to maintain their optimal level of independence and avoid longer term concerns. We provide active rehabilitation to patients in their place of residence to maximise functional potential and support them to remain independent within their homes.

We are seeking an enthusiastic and motivated Physiotherapist to join our busy and dynamic team and promote independence and choice for our service users and their carers.

In return, we'll give you the opportunity to develop specialist skills and enhance your multidisciplinary working; we will provide you with regular training and supervision opportunities as well as the opportunity to develop your leadership skills.

We offer a flexible service which covers extended hours operating 7 days a week, to support our service user needs. Working arrangements include weekends and bank holidays on a rota basis.

  • Be an autonomous practitioner responsible for safe and comprehensive assessment and management of own caseload.
  • Work within your scope of practice.
  • Plan and prioritise workload independently and supervise assistants, support workers and students as required.
  • Support and guide service users in appropriate goal setting.
  • Use clinical reasoning and treatment skills to deliver personalised treatment plans based on evidence based practice.
  • Maintain accurate patient records in line with guidelines.
  • Work as part of a wider MDT to achieve patient goals.
  • Communicate effectively with the wider MDT and case-manage services users as appropriate.
  • Delegate and supervise appropriate tasks and physiotherapy treatment plans to Rehabilitation Workers/ Physiotherapy Assistants/Technicians.
  • Be able to order equipment and ensure its safe and appropriate use, reporting any issues appropriately.
  • Facilitate referrals to other services based on your assessment to ensure holistic patient care.
  • Identify risks and act accordingly to minimise risk to self, service users and colleagues.
  • Actively participate in training; assisting in the delivery of training within the service and to other health professional as appropriate.
  • Actively participate in service development projects as required.
